J. Marion Sims Lecture: Vesico-Vaginal Fistula: Historical Understanding, Medical Ethics, and Modern Sensibilities

The 2017 J. Marion Sims Lecture will be delivered by Dr. L. Lewis Wall, Selina Okin Conner Professor in Arts and Sciences at Washington University in St. Louis. The lecture will address the contributions to fistula surgery made by Dr. Sims in the 19th Century and the controversies that have arisen concerning his operations in the last several decades.

Learning Objectives:

  • Describe the nature of vesico-vaginal fistula arising from prolonged obstructed labor and how it differs from post-hysterectomy fistula
  • Explain the obstetrical context of the 1840s in which these injuries occurred
  • Report what a fistula meant to the women afflicted with this condition in the 19th Century
  • Review the technical problems encountered by surgeons trying to fix these injuries at that time
  • Discuss the modern controversies surrounding Sims’s’ work, including the introduction of anesthesia and the problem of therapeutic experimentation involving slaves
